Long Term Care Programs
Senior and Disability Services offices have information about the long term care services available in your community. Long Term Care programs are designed to ensure client access to and coordination of social and health care resources with an emphasis on client choice, independence and cost effectiveness.
Case managers work with clients and families to develop a plan of care and living arrangements appropriate to each person’s needs. To qualify for Medicaid long term care programs, clients must meet eligibility criteria. Once service and financial eligibility is determined, clients can work with case managers to choose what type of long term care service is best for that individual. Among the services available are in-home care services, placement in community based care settings such as adult foster care, residential care, assisted living, nursing facility care and respite care.
